About the tool

Purpose

This tool provides options for PERTs by matching a patient’s current therapy against equivalent UK licensed products. It also states the enzymatic constituents by product and individual dose.

Included recommendations

The tool includes recommendations for both children and adults.

Recommendations for children have been developed in close consultation with colleagues from the Neonatal and Paediatric Pharmacy Group; recommendations for adults are as published in the Pancreatic Society of Great Britain and Ireland position statement on the PERT shortage.

Creon Micro Pancreatin 60.12 mg Gastro-resistant Granules

Constituents

Each 100mg scoop contains:

  • Amylase 3600 units
  • Lipase 5000 units
  • Protease 200 units

2 scoops of Creon Micro contain:

  • Amylase 7200 units
  • Lipase 10,000 units
  • Protease 400 units

Equivalent products to 2 scoops

2 scoops of Creon Micro could be replaced with:

  • 1 × Creon 10,000 capsules
  • Half a Creon 25,000 capsule
  • 2 × Pancrex V 340mg capsules

Where doses require half capules, this can be achieved by opening the capsule and halving the contents by eye.

The required amount can be given as per the instructions in the BNF, which state:

Manufacturer advises gastro-resistant granules should be mixed with slightly acidic soft food or liquid such as apple juice, and then swallowed immediately without chewing. Capsules containing enteric-coated granules can be opened and the granules administered in the same way. For infants, Creon® Micro granules can be mixed with a small amount of milk on a spoon and administered immediately—granules should not be added to the baby’s bottle.

Update history

  1. Pancrex V powder removed as an alternative as current clinical practice recommends mini-microspheres or enteric coated products as the optimal oral treatment. Available Pancrex V powder should be prioritised for patients receiving enteral feeding.
