About the event

An ‘on the couch’ discussion focusing on upskilling pharmacy professionals in early identification of heart failure and undertaking structured medication reviews in primary care for patients with heart failure.

Why it’s important

Primary care teams have an important role to play in ensuring early and accurate diagnosis of heart failure. Approximately 80% of patients are diagnosed as part of an emergency hospital admission, but half of these present to primary care with symptoms suggestive of heart failure in the months beforehand.

Optimising treatment to manage symptoms of heart failure is important. It can help improve quality of life and reduce hospitalisations.

What was covered

A facilitated discussion between two specialist pharmacists working in primary care and community heart failure teams about why and when primary care pharmacy professionals should ‘think’ heart failure, and hints and tips for undertaking a structured medication review for a patient already diagnosed with heart failure.
