Lelly is a Consultant Pharmacist (Care of older people) with experience of optimising medicines use in frail older people within general practice and community-based multidisciplinary teams. She has championed the development of innovative patient-centred care models that utilise pharmacy teams to facilitate whole systems approach to deprescribing and reducing medicines related problems across health and social care.
